Utah Employment First Partnership

The Utah Employment First Partnership is a commitment among the Utah Department of Work Force Services (DWS), the Division of Services for People with Disabilities (DSPD), and the Utah State Office of Rehabilitation (USOR) to improve state government services focused on persons with disabilities achieving competitive, integrated and community based employment.

The mission of the partnership is to, “To ensure state government services currently offered by the partners emphasize and support competitive, integrated and community based employment”.

TURN Supported Work

~~“TURN Supported Employment services assists individuals with disabilities seeking employment opportunities which are compatible with their unique individual skill-set, and areas of interest. TURN works with our clients to obtain valuable employment skills that easily transfer to relevant work experience with employers offering supported employment opportunities."

Utah Waiver for Technology Dependent, Medically Fragile Individuals

The waiver “provides personal attendant services, skilled nursing respite care, extended home health aide, extended private duty nursing, financial management services, family support, and in-home feeding therapy services to individuals who are medically fragile or who are technology dependent ages 0-20 years who meet a nursing facility level of care.”

Utah Physical Disabilities Waiver

“Provides personal attendant services, financial management services, personal emergency response systems, specialized medical equipment and supplies (monthly fee), specialized medical equipment and supplies (purchase/installation/removal/replacement/repair), and transportation (non-medical) services to individuals ages 65 or older and individuals with physical disabilities ages 18-64 years who meet a nursing facility level of care.”

Utah New Choices Waiver

“Provides adult day care, case management, habilitation, homemaker, respite, supportive maintenance services, consumer preparation services, financial management services, adult residential services, assistive technology devices, attendant care services, caregiver training, chore services, community living services, environmental accessibility adaptations, home delivered meals, medication administration assistance services, personal budget assistance, personal emergency response system, specialized medical equipment/supplies/supplements, and transportation (non-medical) services to individuals ages 65 or older, and individuals with physical or other disabilities ages 18-64 years who meet a nursing facility level of care.”

Utah Division of Services for People with Disabilities Customized Employment Initiative

“The Utah Division of Services for People with Disabilities & Griffin-Hammis Associates is sponsoring a six-session year-long training series resulting in an optional National Certification in Community Employment Services through the Association of Community Rehabilitation Educators (ACRE). The project is working closely with both Covenant Employment Services and Rise, Inc.”

Utah Statewide Transition Plan (HCBS)

This website will provide access to the latest updates on the HCBS Transition Plan for the State of Utah as well as provide a means to sign-up for list-servs, and provide feedback on the Transition Plan, and its associated tools and publications.

UT Community Supports Waiver

This waiver is designed to provide services statewide to help persons with intellectual disabilities or persons with conditions related to intellectual disabilities remain in their homes or other community based settings.  Individuals are able to live as independently as possible with supportive services provided through this waiver program.

Services include “Supported Employment.”

Utah Medicaid State Plan

Utah’s Medicaid State Plan is a written agreement between the state and the federal government.  It describes the scope and nature of the Medicaid program. The state plan also outlines current Medicaid eligibility standards, policies and reimbursement methodologies to ensure the state program receives matching federal funds under Title XIX of the Social Security Act.
