Missouri Comprehensive Waiver, 2023

“The DD Comprehensive Waiver is designed to support individuals with an intellectual or developmental disability to live more independently in their homes and communities. This waiver includes a variety of services that promote community living and integration, including a self-directed option and traditional agency-based service models. The Department of Mental Health, Division of Developmental Disabilities administers this program.”

Missouri Work and Public Benefits

“Many individuals and support professionals report employment decisions are restricted by concerns of losing needed public benefits. Misinformation and the shortage of informed professionals lead many individuals to choose unemployment, ongoing poverty and/or compromise full potential to avoid losing benefits. There are several work incentives to help people with disabilities achieve greater independence by helping them to take advantage of employment opportunities. Work incentives are intended to help individuals go to work and minimize the risk of losing their needed benefits. There are many scenarios of how an individual can work, increase his/her income and retain needed benefits.”

 

Missouri HCBS Transition Plan, amended 2019

“In January 2014, the Centers for Medicare & Medicaid Services (CMS) published the federal Home and Community-Based Services (HCBS) regulation, commonly known as the “HCBS Final Rule.” The purpose of the HCBS Final Rule is to ensure that Medicaid HCBS participants have the benefit of community living and the opportunity to receive services in the most appropriate, integrated setting possible. Since it was published, Missouri has been engaged in a thoughtful process to understand and meet the letter and intent of the HCBS Final Rule with its 10 Medicaid HCBS Waiver programs…
…This Statewide Transition Plan incorporates all 10 DHSS and DMH waivers and has been jointly developed by MHD, DMH and DHSS, herein referred to as the State. The transition plan as outlined below has sections that apply exclusively to waivers operated by DHSS, waivers operated by DMH, and those operated by both state agencies. It provides a high level overview of Missouri’s HCBS Medicaid programs, outlines the details of the steps taken to ensure compliance by March 2019, and outlines the public comment process which ensures input from self-advocates, families, advocacy organizations, and providers.”

Missouri Division of Developmental Disabilities Individual Support Plan Guide (2014)

“The Division of Developmental Disabilities requires that each individual eligible for Division Supports have an Individual Support Plan. Individuals, their families, providers and facilitators who write plans in cooperation with all individuals receiving supports from the Division shall use this guide. Individual support planning encourages a team approach to involve the individual and community networks in planning for the future. The process involves developing a vision for the future, while coordinating resources and supports to make the vision a reality. The Center for Medicare and Medicaid Supports (CMS) outcome for “participant-centered support planning and delivery” clarifies: “Supports are planned and effectively implemented in accordance with each participant’s unique needs, expressed preferences and decisions concerning his/her life in the community.” The Individual Support Plan is an investment in an individual’s life and is driven by the individual, what is important to them as designed through outcomes identified in the support plan.”

Missouri’s Cooperative Work Experience Program (COOP)

“Successful transition planning for students with disabilities involves looking ahead, into the student’s future, preparing not just for graduation but also for life after graduation. One of the most important aspects of a student’s future involves employment. However, students often do not have any idea what type of work they would like to do or what type of career options are available. One of the best ways that a student can learn about different careers and employment is to be exposed to and participate in cooperative work experiences prior to graduation or exit from school. Cooperative work experiences provide students with direct exposure to and hands-on exploration of a variety of careers, assisting them in defining vocational choices and setting employment goals that actually seem attainable.”

Employment provides income, a sense of belonging, mental and tangible benefits to a human, and status. As such, it is the position of the Governor’s Council that employment policies, practices, and implementation be inclusive and competitive”

Missouri HCBS Statewide Transition Plan, 2016

“Missouri administers 10 Home and Community-Based Waivers through the single State Medicaid agency, the Department of Social Services, MO HealthNet Division (MHD). The day-to-day operation of the waivers is through formal cooperative agreements with the Missouri Department of Mental Health (DMH) and the Missouri Department of Health and Senior Services (DHSS). The Department of Health and Senior Services and the Department of Mental Health are the operational entities for the waivers. Missouri Medicaid Audit and Compliance (MMAC) is the unit within the Department of Social Services (DSS) charged with administering and maintaining Medicaid Title XIX audit and compliance initiatives, including utilization of Medicaid services and provider enrollment functions. MMAC will participate in the transition plan as described below.”

Missouri Governor’s Council on Disability: Missouri Interagency Transition Team (MITT)

“The Missouri Interagency Transition Team (MITT) formed in 2007 by the Office of Special Education at the Missouri Department of Elementary & Secondary Education to increase interagency collaboration at the state, regional, and local levels. This state transition team has met quarterly to address data-driven goals for improvement and collaboration with the shared vision of improving employment, independent living, and postsecondary education outcomes for Missouri students with disabilities…

…The resources listed on this site are for those individuals with disabilities who are looking for any level of independent living whether from school age to adulthood. It could be partial independence such as finding a job, or going to school, to full independent living, living on your own and taking care of yourself with limited or no assistance.”

Missouri Dept. of Developmental Disabilities: Empowering through Employment, 2016

“In October 2016, the Division of Developmental Disabilities (DDD) launched the Empowering through Employment Initiative to assist the growing number of individuals who express an interest in community-based employment.  According to National Core Indicators, 51% of individuals receiving home and community-based services express an interest to work in the community; however, fewer than 25% had a goal in their annual plan to support this interest and only 3% of individuals on a monthly basis have services authorized to assist with their employment pathway. “
