~~“Vocational Rehabilitation provides Transition Services to eligible students with disabilities as they transition from high school to adulthood and employment. Our goal is to provide students (ages 14-24) with a smooth transition, holistic team approach, and individualized activities and services that are necessary and reasonable to help prepare for, obtain, and maintain employment. We partner with members of the school, family, and/or IEP team, as well as other adult service agencies to ensure that transition-aged youth with disabilities have the supports needed to successfully transition from high school to adulthood, independence, and employment.”
