West Virginia Medicaid Money Follows the Person – Take me home

“Take Me Home (TMH) Transition Program
​​​​​​​​​The Take Me Home (TMH) Transition Program is a program of the West Virginia Bureau for Medical S​ervices (BMS). The purpose of TMH is to identify qualifying residents of long-term care facilities who wish to return to their own homes and apartments in the community and provide them with supports and services they need to do so. The TMH Transition Program is supported by West Virginia’s Money Follows the Person (MFP) Rebalancing Demonstration Grant.

Money Follows the Person (MFP) Rebalancing Initiative​ Overview
West Virginia was awarded a Money Follows the Person MFP demonstration grant by the Centers for the Medicare and Medicaid Services (CMS) in 2011. West Virginia’s MFP program is called Take Me Home (TMH), West Virginia.

Currently, 3​​3 states and the District of Columbia are participating in the MFP demonstration. The purpose of MFP is to support ​​state Medicaid programs in providing people with long-term care needs a greater choice of where to live and receive needed services and supports.”
