“The federal Centers for Medicare & Medicaid Services (CMS) requires states to submit a transition plan describing the state’s planned initiatives and activities for achieving compliance with the federal HCBS Settings Rule. HHSC has received initial approval of its transition plan and has submitted an updated version of the plan to CMS for final approval.”
“Home Care programs are Medicaid-funded long-term-care programs designed to help eligible elderly or disabled individuals remain safely at home, rather than in a nursing home. Home Care programs have different eligibility requirements, but they all require that you be eligible for Medicaid.”

A federal option, called Community First Choice, allows states to provide home and community-based attendant services and supports to Medicaid recipients with disabilities. This option provides states with a 6 percent increase in federal matching funds for Medicaid for these services.”
“Community first choice (CFC) is a Washington apple health state plan benefit authorized under Section 1915(k) of the Social Security Act. CFC enables the agency and its contracted entities to deliver person-centered home and community based long-term services and supports (LTSS) to Medicaid-eligible people who meet the institutional level of care.”
This waiver provides “respite, transition case management, assistive technology and service evaluation, environmental accessibility adaptations for medically fragile individuals ages 0-20”.
“Provides community support, home support 1/4 hour, respite, work support-group, financial management services, individual goods and services, supports brokerage, assistive technology, career planning, employment specialist services, home accessibility adaptations, home support-remote support, non-medical transportation, shared living, and work support-individual services to individuals with autism or intellectual disabilities ages 18 or older who meet an ICF/IID level of care. This waiver operates with a concurrent 1915(b)(4) authority.”
