Indiana Medicaid State Plan

The Indiana Medicaid state plan details the agreement between the state and the Federal government. It describes how Indiana administers its Medicaid program and explains how the state will abide by Federal rules.  It also explains how Indiana may claim Federal matching funds for its program activities.

Indiana Money Follows the Person

“The MFP program is funded through a grant from the federal agency, Centers for Medicare and Medicaid Services. The MFP program was developed to help states move individuals from institutional settings to home and community-based settings. Indiana was approved for the MFP program in 2007 and since that time has focused on assisting eligible persons to leave institutional care by providing services for individuals to live safely in their community.”

Ohio Money Follows the Person

“The Money Follows the Person (MFP) Rebalancing Demonstration Grant helps states rebalance their Medicaid long-term care systems. Over 75,151 people with chronic conditions and disabilities have transitioned from  institutions back into the community through MFP programs as of December 2016. The Affordable Care Act of 2010 strengthened and expanded the MFP program allowing more states to apply. There are currently forty-three states and the District of Columbia participating in the demonstration.”

New York Medicaid Infrastructure Grant MIG-RATS

Medicaid Buy-In Program For Working People with Disabilities (MBI-WPD) implemented July 2003 under the federal authority of  TWWIIA Basic + Medical Improvement.

The Medicaid Infrastructure Grant Research Assistance to States (MIG-RATS) Center launched a website to provide resources and support to states implementing MIGs. The website is designed to help staff find research reports and resources, learn about MIG-RATS activities and initiatives, and connect with MIG researchers. The website includes info on topics such as Medicaid Buy-In programs, outreach and marketing, and youth in transition and also provides links to tools and a calendar of events.

Arkansas Community and Employment Support Waiver (0188.R06.00)

Provides case management, respite, supported employment, supportive living, specialized medical supplies, adaptive equipment, community transition, consultation, crisis intervention, environmental mods, supplemental support for individuals with autism, IIDR, DD ages 0 – no max age

Iowa Home and Community Based (HCBS) Waivers Program

“In Iowa, there are seven programs called Home-and Community-Based Services (HCBS) Waivers. These waivers help people stay in their own homes or communities instead of going to a medical institution.

These programs also provide funding and personalized support for individuals who qualify, allowing them to receive the care they need while remaining in a familiar environment.

Rhode Island Medicaid Buy-In Program

“Medicaid Programs For Working People With Disabilities
The Medicaid Programs for Working People with Disabilities is for people who fall within two categories:

  • Ticket to Work for ages 16-64
  • Sherlock Plan for ages 65 and older”

Virginia Olmstead Settlement Agreement (2012)

“The Commonwealth shall establish a state policy on Employment First for the target population and include a term in the CSB Performance Contract requiring application of this policy. The Employment First policy shall, at a minimum, be based on the following principles: (1) individual supported employment in integrated work settings is the first and priority service option for individuals with intellectual or developmental disabilities receiving day program or employment services from or funded by the Commonwealth; (2) the goal of employment services is to support individuals in integrated work settings where they are paid minimum or competitive wages; and (3) employment services and goals must be developed and discussed at least annually through a person-centered planning process and included in ISPs. The Commonwealth shall have at least one employment service coordinator to monitor implementation of Employment First practices for individuals in the target population

New York Long Term Home Health Care Waiver (terminated 2016)

WAIVER TERMINATED – NY Long Term Home Health Care Program Waiver (0034.R06.00)

WAIVER TERMINATED on 5/27/2016 – Provided respite, assistive technology, community transitional services, congregate and home delivered meals, environmental mods, home and community support services, home maintenance, medical social services, moving assistance, nutritional counseling/education services, respiratory therapy, social day care transportation, and social day care services to individuals ages 65 or older and individuals with physical disabilities ages 0-64 years who met a nursing facility level of care.  This waiver operated with a concurrent 1115 authority.

New York OPWDD Comprehensive Medicaid Waiver

“Provides day habilitation, live-in caregiver, prevocational services, residential habilitation, respite, supported employment, community transition services, fiscal intermediary, individual directed goods and services, support brokerage, assistive technology/adaptive devices, community habilitation, environmental modifications (home accessibility), family education and training, intensive behavioral services, pathway to employment, and vehicle modification services to individuals with autism, intellectual disabilities, or developmental disabilities ages 0 or older who meet an ICF/IID level of care. This waiver operates with a concurrent 1915(a)(1)(a) and 1915(b)(4) authority.”
