“Provides day habilitation, live-in caregiver, prevocational services, residential habilitation, respite, supported employment, community transition services, fiscal intermediary, individual directed goods and services, support brokerage, assistive technology/adaptive devices, community habilitation, environmental modifications (home accessibility), family education and training, intensive behavioral services, pathway to employment, and vehicle modification services to individuals with autism, intellectual disabilities, or developmental disabilities ages 0 or older who meet an ICF/IID level of care. This waiver operates with a concurrent 1915(a)(1)(a) and 1915(b)(4) authority.”

Rhode Island Money Follows the Person Grant: The Rhode to Home

“Rhode Island’s ‘The Rhode to Home’ is a new program that will align with other State efforts to re-balance RI’s long-term care system. The Rhode to Home will provide support to transition eligible individuals who are in a qualified institutional setting for 90 days or more to home and community-based settings. It’s also referred to as Money Follows the Person or MFP. This demonstration project will assist individuals transition to and successfully remain in the community, with appropriate supports, so that they can experience more independence and a better quality of life. Participation is voluntary.”

Oklahoma Living Choice/ Money Follows the Person

“Oklahoma’s Living Choice program gives eligible Oklahomans the opportunity to live in their own homes in a community setting while receiving the necessary services and support to meet their needs. The Medicaid program is designed to transform the current long-term care system by promoting home and community-based care instead of institutional care.

With the right support, many individuals can regain their independence as active, engaged community members. Living Choice offers various community-based services to help make the move successful.

Living Choice has transitioned approximately 1,000 Oklahomans and saved the state over $9 million.”

South Dakota Money Follows the Person
