Texas Community Living Assistance and Support Services (CLASS) Waiver

“CLASS provides home- and community-based services to people with related conditions as a cost-effective alternative to placement in an Intermediate Care Facilities for Individuals with an Intellectual Disability or Related Conditions (ICF-IID).”

This waiver “provides case management, prevocational services, residential habilitation, respite (in-home and out-of-home), supported employment, adaptive aids, dental treatment, dietary, nursing, occupational therapy, physical therapy, prescribed drugs, speech and language pathology, financial management services, support consultation, auditory integration training/auditory enhancement training, behavioral support, cognitive rehabilitation therapy, continued family services, employment assistance, minor home modifications, specialized therapies, support family services, and transition assistance services to individuals with developmental disabilities ages 0 or older who meet an ICF/IID level of care.”

Texas Home and Community Based Services (HCBS)

“What are Home and Community Based Services (HCBS) Rules?

People who receive Medicaid home- and community-based services (HCBS) soon will have the same opportunities as people who don’t receive those services. Thanks to new federal Centers for Medicare & Medicaid Services (CMS) rule, people who receive Medicaid HCBS will have the right to live in the most integrated setting and have full access to community living. Lifestyle decisions will be made with each person based on his or her preferences, also known as person-centered planning. The federal rules were effective March 17, 2014.

People getting Medicaid HCBS have the right to:

  • Seek employment
  • Work in competitive or integrated settings
  • Engage in community life
  • Control their personal resources
  • Receive services in the community”

Utah HCBS Waivers

DSPD provides a wide range of services intended to help people with disabilities participate more fully in their communities and lead self-determined lives. The type of services available depends on which waiver the person qualifies for. These services are primarily funded through Medicaid waivers, which are paid for by the federal government and appropriations by the Utah Legislature.

Utah Acquired Brain Injury (ABI) Waiver

“Provides ABI waiver support coordination, center-based prevocational services, day supports, homemaker, residential habilitation, respite, supported employment, consumer preparation services, financial management services, behavior consultation I, behavior consultation II, behavior consultation III, chore services, community transition service, companion services, environmental adaptations (home), environmental adaptations (vehicle), extended living supports, massage therapy, personal budget assistance, personal emergency response system, professional medication monitoring, specialized medical equipment/supplies/assistive technology (purchase), specialized medical equipment/supplies/assistive technology (monthly fee), supported living, and transportation (non-medical) services to individuals with brain injury ages 18 or older who meet a nursing facility level of care.”

Utah Waiver for Individuals Age 65 or Older

“Provides adult day health, case management, homemaker, respite (LTC facility), enhanced state plan supportive maintenance home health aide services, financial management services, adult companion services, chore services, community living services, environmental accessibility adaptations, medication reminder systems, personal attendant services, personal budget assistance, personal emergency response systems (installation/testing/removal), personal emergency response systems (purchase/rental/repair), personal emergency response systems (response center service), specialized medical equipment/supplies/assistive technology, supplemental meals, and transportation (non-medical) services to individuals ages 65 or older who meet a nursing facility level of care.”

Missouri HealthNet Eligibility for People with Disabilities

“Disability Definition for MO HealthNet for the Disabled (MHD)
The definition of disability for MO HealthNet is the same definition used to determine disability for Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI) benefits.

The Social Security Act defines disability as:

  • the inability to engage in any substantial gainful activity (SGA) due to a physical or mental impairment(s) which:
  • can be expected to result in death or
  • has lasted or can be expected to last for a continuous period of at least 12 months.
    SGA is the ability to be employed and earn over $1,180 per month (2018).”

Utah New Choices Waiver

“Provides adult day care, case management, habilitation, homemaker, respite, supportive maintenance services, consumer preparation services, financial management services, adult residential services, assistive technology devices, attendant care services, caregiver training, chore services, community living services, environmental accessibility adaptations, home delivered meals, medication administration assistance services, personal budget assistance, personal emergency response system, specialized medical equipment/supplies/supplements, and transportation (non-medical) services to individuals ages 65 or older, and individuals with physical or other disabilities ages 18-64 years who meet a nursing facility level of care.”

Utah Physical Disabilities Waiver

“Provides personal attendant services, financial management services, personal emergency response systems, specialized medical equipment and supplies (monthly fee), specialized medical equipment and supplies (purchase/installation/removal/replacement/repair), and transportation (non-medical) services to individuals ages 65 or older and individuals with physical disabilities ages 18-64 years who meet a nursing facility level of care.”

Utah Waiver for Technology Dependent, Medically Fragile Individuals

The waiver “provides personal attendant services, skilled nursing respite care, extended home health aide, extended private duty nursing, financial management services, family support, and in-home feeding therapy services to individuals who are medically fragile or who are technology dependent ages 0-20 years who meet a nursing facility level of care.”

Washington Core Waiver

“Provides community inclusion, individual supported employment/group supported employment, residential habilitation, respite, occupational therapy, physical therapy, speech/hearing/language services, assistive technology, community engagement, community transition, environmental adaptations, extermination of bed bugs, individualized technical assistance, life skills, remote supports, risk assessment, skilled nursing, specialized equipment and supplies, specialized habilitation, stabilization services (crisis diversion bed), stabilization services (life skills), stabilization services (staff/family consultation), stabilization services (specialized habilitation), staff/family consultation services, supported parenting, transportation, and wellness education services to individuals with autism, intellectual disabilities, or developmental disabilities ages 0 or older who meet an ICF/IID level of care.  This waiver operates with a concurrent 1915(b)(4) authority.”
