Kansas Money Follows the Person (2017)

“If you live in a nursing home, the Money Follows the Person (MFP) program may be able to help you move home. The state of Kansas received grant money from The Centers for Medicare and Medicaid Services (CMS). This money is used to help people move out of nursing homes. The MFP provides Transition Coordination Services. That means the program will pay someone to help you with each step. It covers things like:
• Finding an apartment or assisted living facility.
• Filling out apartment applications.
• Getting all the legal documents you need to
move into the community.
• Paying up to a certain amount for household
items. You may need furniture, kitchen utensils,
pots and pans, and bedding.
• Paying up to a certain amount to make changes
to your home. You may need a wheelchair ramp
or wider door ways. The bathroom may need
more space for grab bars for the tub or toilet.
• Paying security deposits.
• Finding the personal care that you need.
Care may include help with bathing, going
to the bathroom, meals and housekeeping.”

Mississippi Elderly and Disabled Waiver

“The Elderly and Disabled Waiver program is administered and operated by the Office of Long Term Care. Case Management services are provided by the Planning and Development Districts. The case management team is composed of a registered nurse and a licensed social worker who are responsible for identifying, screening and completing an assessment on individuals in need of at-home services.”

Delaware Lifespan Waiver

“The Lifespan Waiver provides eligible Delawareans access to Medicaid-approved services through the 1915(c) Home and Community Based Services Waiver. Individuals on the waiver are supported to live more independently in their homes and communities through services that promote and support community living.”

Hawaii Medicaid IDD Waiver Application (2017)

“The State of Hawaii requests approval for an amendment to the following Medicaid home and community-based services waiver…. Program Title: HCB Services for People with Intellectual and Developmental Disabilities (I/DD Waiver).  Amendment Number: HI.0013.R07.01. Approved Effective Date of Waiver being Amended: 07/01/16.

This is a technical amendment to address several items that were not included in the waiver renewal approved effective July 1, 2016. The new rate methodology and changes to services are designed to support implementation of the Home and Community Based Services Final Rule for Community Integration. This amendment also includes a multi-year phase-in using the Supports Intensity Scale for Adults to assess participants’ support needs to inform the person-centered process.”

Hawaii Medicaid Phase-In Timing for New Rates

“This document is a set of tables noting what services will be available for all of the participants in the program or different “cohorts” (Cohort 1 – Participants residing in certified or licensed settings; Cohort 2 – Participants living independently, in family homes, or unlicensed settings AND receiving Adult Day Health; or Cohort 3 – Participants living independently, in family homes, or unlicensed settings and NOT receiving Adult Day Health).”

Alaska Medicaid Transition Period for Compliance with HCBS Criteria (2017)

“A CMS Informational Bulletin was released on May 9, 2017, indicating that the transition period for complying with home and community-based settings criteria is extended until March 17, 2022. In the Bulletin, CMS indicated that states may still choose to meet the original March 17, 2019 deadline. The State of Alaska values the person-centered transformation that will result from providers meeting the settings criteria, and acknowledges the progress made to bring Alaska’s providers into compliance. Accordingly, Alaska will retain the original March 17, 2019 deadline for settings to become compliant with the CMS home and community-based settings criteria.”

Washington Medicaid Transformation Project

MTP is Washington State’s Section 1115 Medicaid demonstration waiver between the Health Care Authority (HCA) and Centers for Medicare & Medicaid Services (CMS). MTP allows our state to create and continue to develop projects, activities, and services that improve Washington’s health care system.

On June 30, 2023, CMS approved MTP to continue for five more years. Our state’s MTP renewal, called MTP 2.0, will help widen our reach to provide more programs, services, and supports to our most vulnerable populations.

Hawaii Medicaid Going Home Plus Project FAQs

“The Going Home Plus (GHP) project helps residents who have been living in hospitals, nursing facilities, and ICF/ID facilities move back into the community. For those residents who choose to live in the community, the GHP project will assist in finding housing (if the resident does not have a home to return to) and services (for example, help with cooking and bathing) GHP is funded by a federal demonstration grant from the Centers on Medicare and Medicaid Services (CMS) called Money Follows the Person. Hawai‘i’s GHP began in 2008 and will continue through 2020.”
